We use Dr. Chrono Payments powered by paySimple and Stripe....paysimple has a way to do it outside of Dr. Chrono, but I want it to be integrated and simple.... ;) I hope your suggestion works. Thanks!

Create a "RECURRING APPOINTMENT" and a "CUSTOM CODE", applied to that appointment (worth creating a appt/billing profiles for this)... scheduled for the 1st of every month...
*who do you use for payment processing? Square has a recurring invoice/subscription option
